Insurance & Pricing Information

Here at Exemplify Health Center, we desire to be open and upfront about what each service may cost you and which is billable or not billable to insurance. This allows you to make the best decision for your health care.

DIAGNOSTIC TESTING (LAB WORK)

• Lab work is based on the individual and varies greatly in price. Exemplify Health Center does not up-charge for lab testing. We charge exactly what the labs charge us.
• Some lab work is billable to insurance and some is not.
• Pricing and insurance information will be gone over by a doctor or staff member when the test kit or order for blood work is given.
• Consultations to go over lab results, discuss plan of care, or make recommendations ARE NOT included in the price of the lab test.

CHIROPRACTIC SERVICES THAT MAY BE BILLABLE TO INSURANCE WHEN THERE IS A FUNCTIONAL NEED

• Please note that just because a service is billable to insurance does not mean it will be covered by your insurance provider. Insurances require a musculoskeletal complaint that directly interferes with your daily activity.
• Currently, we are not participating providers in any insurance networks with the exception of Medicare. All chiropractic services that are medically necessary will be billed to your insurance as an out-of-network provider as a courtesy to you.

CHARGES & PRICE RANGE

• Exams (New Patient Exams & Re-Exams): $40 - $200
• X-Rays: $50 - $260
• Chiropractic Adjustments: $30 - $70
• Therapy: $20 - $50
• Custom Foot Orthotics: $300 - $500

SERVICES NOT BILLABLE TO INSURANCE

• Insurance companies do not allow any of the following services to be billed. If you have an HSA (Health Savings Account) they may or may not allow for reimbursement. Please check with your HSA to determine what they allow to be purchased with HSA money.
• Consultation and nutrition pricing is based on the individual. Once test results are received a billing specialist will meet with you to determine approximate pricing of your care plan and offer discounted pricing on bundled services. Contact the billing department for more information.

CHARGES & PRICE RANGE

• Supplements & Super Foods: $10 - $125 per supplement
• Patient Education / Nutritional Counseling: $200 - $300
• Consultations / Phone Consultations: $200 - $300
• Test Result: $200 - $300
